Insurance
We accept the following insurances in our office: Aetna, Anthem, Cigna, Medicaid, and United Health Care along with many others. Please check with your insurance company to make certain we are providers on your plan.
**We currently do not accept:
1. New Medicaid patients
2. Workers' Compensation claims, unless referred for a specialized treatment
3. Any managed medicaid (CareSource, Molina)
Payments/CoPays
Copays, deductibles, non-covered services and non-participating insurances are to be paid at the time of the visit. The requirements of your insurance contract state that you are to pay copays at the time of the visit.
